A Dedicated Team

The Laguna Honda Rehabilitation Center is dedicated to helping people fulfill their potential.

Laguna Honda is an acute care hospital as well as a rehabilitation center and skilled nursing facility that utilizes a team-based approach. Your Resident Care Team (RCT) is led by licensed clinical professionals who will contribute to your individual care plan. Staff physicians are available on campus 24 hours a day, seven days a week. Nursing staff includes home health aides (HHA), patient care assistants (PCA), certified nursing assistants (CNA), licensed vocational nurses (LVN), and registered nurses (RN). Other health care professionals, such as psychiatrists, psychologists, physical therapists, occupational therapists, speech therapists, audiologists, clinical nurse specialists and clinical pharmacists are available to provide consultation on an as-needed basis. The Care Team also includes representation from the departments of Activity Therapy, Clinical Nutrition and Social Services. These individuals are highly trained in specific areas that are vital to your health needs and enhance your experience at Laguna Honda.

We strive to prepare our clients to return home with the physical abilities and resources they need to live full and satisfying lives.

A Center of Excellence

The healing environment of our 62-acre campus, plus our innovative programming and our state-of-the- art facility opening in 2010, combine to give clients the resources they need to reclaim their momentum after a life-changing diagnosis.

The architecture of our new building is scientifically designed to promote healing, with ample outdoor space, airy and well lit common areas, a specially designed outdoor terrain park, top of the line therapeutic equipment, and 2 new pools!

The Laguna Honda model of care applies skill from a range of professional disciplines to treat physical and cognitive symptoms.

Laguna Honda doctors certified in rehabilitative medicine work together with specially trained rehabilitation nurses, neuropsychologists, physical and occupational therapists, speech and language pathologists, audiologists, dietitians, social workers, and others to prepare a treatment plan that meets the needs of each person we care for.

Our staff is uniquely qualified to address complex needs with special certifications in geriatrics, neurologic rehabilitation, aquatics, low vision, and hand therapy, just to name a few.

Our commitment to evidence-based therapeutic excellence, client self-sufficiency, and quality of life extends to everything we do.  We love our work, and that passion creates a warm and stimulating environment of care.

Meeting Your Needs

Applicants to the Laguna Honda Rehabilitation Center must meet two criteria: a condition that demonstrates a need for intensive rehabilitation, and a willingness and ability to participate in the program.

Our clients can count on us to be their partners in recovery.  We know that the challenges of reclaiming function are among the most difficult anyone can face.  That’s why we respect each client’s wishes and encourage discovery of new possibilities.  We tailor our work to meet the unique needs of each person.   Clients are considered part of the interdisciplinary team, and they meet weekly with team members to collaborate on their care plans.

Caring For The Whole Person

Our interdisciplinary team provides comprehensive care in an environment that nurtures each individual as a whole person.  Our approach is to provide focused care that promotes full community integration whenever possible.

When clients graduate from the rehabilitation program at Laguna Honda they are able to move back into their lives with their aspirations renewed.
