HEALTH OFFICER ORDER NO. C19-11
(REVISION OF PUBLIC HEALTH EMERGENCY ORDER)

The purpose of this notice is to advise Laguna Honda Hospital and Rehabilitation Center (“Laguna Honda”), its Residents, and the families and loved ones of those Residents of an interim revision to Health Officer Order No. C19-11, issued March 24, 2020, at the start of the pandemic. Conditions in the Bay Area and across the country have changed substantially since the start of the pandemic. Although not every Resident at Laguna Honda is willing or able to receive one of the vaccines that are effectives against Coronavirus Disease 2019 (“COVID-19”), a majority of residents and Staff at Laguna Honda have been fully vaccinated, and the vaccination rates in the Bay Area are high and the current COVID-19 infection rates in the community have dropped significantly. Collectively, these factors mean it is substantially safer for Laguna Honda Residents to leave the facility and go into the community—especially when those Residents are fully vaccinated—subject to reasonable controls to protect those at Laguna Honda who remain at-risk for COVID-19. And to that end, all Residents and Staff are strongly urged to be vaccinated against COVID-19 if they have not already done so.
This notice modifies Order C19-11, effective immediately, to allow the Laguna Honda administration, in consultation with the Health Officer, to allow Residents to leave the Protective Quarantine created by Order No. C19-11 subject to the following requirements:
- Laguna Honda administration must adopt a plan regarding how to allow Residents to leave Laguna Honda temporarily (to go “Out On Pass”), and that plan must take into account the vaccination status of the Resident who is going Out On Pass.
- The Laguna Honda Out On Pass plan must include protocols to protect Residents who are not fully vaccinated from being put at risk by any Resident who is not fully vaccinated who returns from being Out On Pass. To that end, Laguna Honda is allowed to make use of isolation and quarantine procedures, as appropriate, for any Resident returning from being Out On Pass, as well as any other appropriate restrictions. Anyone who goes Out On Pass must be notified of the procedures prior to being allowed Out On Pass. This allowance is in line with Health Officer Order No. C19-11, as well as the separate Facility Quarantine, Isolation, and Testing Order issued regarding Laguna Honda on March 29, 2020, both of which remain in full effect (subject to the revisions of this Notice).
- Laguna Honda may immediately revoke or revise its Out On Pass plan in order to ensure the safety of its Residents, and Laguna Honda is required to keep the Health Officer advised of any changes to its Out On Pass program
In the coming weeks, the Health Officer will be substantially updating Order No. C19-11 consistent with current best practices in light of the low SARS-CoV-2 infection rates and wide availability of vaccination for COVID-19 in the City and County of San Francisco.

A copy of this Notice must be posted on the Laguna Honda website, and a copy must be provided to any Resident, support person of a Resident, or member of the public on request.
